1. Who we are

Cyber Medical Credentialing provides provider credentialing and payer enrollment services to healthcare providers. Our office is at 1881 N University Dr, Suite 105, Coral Springs, FL 33071.

This policy covers information collected through this website, cmcredentialing.com, and when you contact us or book an appointment.

2. What this policy does not cover

Provider information we handle under a client agreement — credentialing files, licensure documents, malpractice history and the rest — is governed by that agreement and by any applicable Business Associate Agreement, not by this website policy.

3. Information we collect

When you browse this website. This site has no contact form, no user accounts, and sets no analytics or advertising cookies. Our host keeps standard server logs — IP address, browser type, pages requested and timestamps — used for security and operations.

When you book an appointment. Our booking page is hosted by Microsoft Bookings. We receive the details you enter: name, email address, phone number and anything you write in the other fields.

When you call or email us. We keep your message, our reply and your contact details so we can respond and maintain a record.

Fonts. This site loads its typeface from Google Fonts, which makes your IP address visible to Google. We receive nothing from this.

4. How we use it

• To respond to your inquiry and hold the appointment you booked
• To provide, administer and invoice for our services
• To keep our systems and our clients' information secure
• To meet legal, regulatory and contractual obligations

We do not sell personal information and we do not use it for advertising.

5. Who we share it with

• Service providers operating systems on our behalf — our host, and Microsoft for email and scheduling.
• Payers and facilities, where submitting an application on a provider's behalf requires it, and only with that provider's authorization.
• Professional advisors and authorities, where relevant or legally required.

6. Cookies

This website sets no cookies and uses no tracking or analytics scripts. Our booking page is operated by Microsoft and may set its own.

7. How we protect information

Credentialing files contain sensitive professional and personal information about providers. We restrict access to staff who need it, transmit documents through secured channels, and retain them only as long as our agreements and applicable law require.

No method of transmission or storage is completely secure. Email in particular is not a secure channel.

8. Your choices

You may ask us for a copy of the personal information we hold about you, to correct it, or to delete it. Write to info@sixequation.com and we will respond within a reasonable period. We may need to verify your identity first.

If you are a California resident, you have additional rights under the California Consumer Privacy Act. We do not sell personal information, so there is nothing to opt out of.

9. Children

This website is intended for healthcare providers and their staff, and is not directed at children.

10. Changes

We may update this policy. The effective date above shows when it last changed.
